New User Registration Form

As a byproduct of the rationalization of the CERN's Human Resource Database (HR) and the IT Computer Centre Data Base (CCDB), we have revised the User Registration form to be filled in for new users of Central Computing facilities.

The main changes are:

• There are no longer two separate forms ("green" and "yellow") to distinguish between "General Users" and "Outside Users". The new form is white, thus compatible with your own printed copies or the faxed forms.
• There is additional mandatory information which has to be filled in, in order to ensure correct matching with the registration in HR. Please note, as indicated in the form, that such information will be treated confidentially and only for the purpose stated above.
